We are excited to have you join us at the Health System Forum. Please fill out the form below to register. You can register multiple attendees by clicking the   button. When registering on behalf of others, you will need to enter their individual email addresses and mailing addresses. Each registered attendee will receive a confirmation email and additional information will be emailed closer to the event.

Registration will close at 12:00 p.m. EST on Monday, February 8, 2021.

Preferred payment is via credit card. If paying via PO, it must be received by February 8, 2021 to secure your spot.

Important note: Each participant will be mailed a Forum toolkit full of resources and supplies to support learning during the event. When registering, please provide the mailing address where you would like the Forum toolkit sent. We will try our best but cannot guarantee toolkit delivery on time for registrations received after February 1.

If you are interested in the Forum, but cannot attend due to a time conflict or can only attend part of the day, please register as usual using the registration form. We will be releasing recordings after the event and will send you a toolkit.

Virtual Forum Cancellation, Substitutions & Refund Policy

  • Payment is required in full prior to the start of the Virtual Forum.
  • Full refund will be given if cancellation is received at least 14 days prior to the start of the Virtual Forum.
  • No refund will be given if cancellation is received less than 14 days prior to the Virtual Forum, but you will receive a credit toward a future Virtual Forum or you may send a substitute in your place in lieu of a credit.
